如何注册

To register with the 无障碍服务办公室 and request accommodations for your 残疾/disabilities, 请填写 申请/医疗证明表格. 在您提交表单之后, the Accessibility Services Coordinator will notify you of any approved accommodations. 请查看 这个视频 举个例子.

请求无障碍服务的指南

Please follow the guidelines below to register with the 无障碍服务办公室 and request accommodations for your 残疾/disabilities.  Submitted information should be current and from a medical provider who is familiar with your 残疾. After you and your medical provider have completed and returned the required documentation, you will be notified by the Accessibility Services Coordinator of any approved accommodations.

1. 残疾证明文件

您的文档应包括但不限于:
诊断, 确认此诊断的测试和结果, 以及医生最后一次和你联系的日期
与此病相关的功能限制或症状
描述这种情况如何影响你的学业和/或身体
住宿建议(一).e. 延长测试时间)和这些调整的基本原理 
残疾的严重程度和/或预期进展
2. 向你的学术顾问注册课程

If accommodations such as interpreters or CPrint are needed for meeting with your academic advisor, 联系无障碍服务协调员. 学生 who wish to receive accommodations are encouraged to register for classes as early as possible to allow adequate time to arrange support services and accommodations.

3. Arrange a Strong Start appointment with the Accessibility Services Coordinator to go over your schedule of classes, 可用的资源,  讨论批准的住宿条件.

This should be done as early as possible to ensure that we can set up accommodations before you start your courses.

残疾的定义 A person has a 残疾 if he or she has a physical or mental impairment that substantially limits one or more “major life activities.“主要的生命活动包括行走等功能, 看到, 听力, 呼吸, 学习, 工作, 照顾好自己, 执行手工任务. 

由合格的专业人员诊断 学生 requesting services should provide information of their 残疾 by a qualified professional. A “qualified professional” has significant experience diagnosing and/or treating a given disorder. The information should provide a diagnosis of the type of 残疾 and a specific description of how the 残疾 interferes with educational achievement. 

保密声明 Disability information is shared only on a limited basis within the University and only when t在这里 is compelling need to communicate with faculty and staff in order to achieve implementation of reasonable accommodations. 

自我识别残疾 才有资格住宿, you must self-identify as a person with a 残疾 or impairment to the 无障碍服务办公室. 披露这些信息是自愿的, 提供这些信息不会产生任何不良后果.
